Upload Picture in a Form

Hi! Could anyone please help me about this?
In my BV form. I create a file upload for picture and I want to create only two or four pictures with a certain maximum size are allowed to be upload. How can I do it?

Of course if you want to upload more than one pics at a time, you have to repeat the uploading part of the script as weel as the URL creating part of it
