Cannot make form to work!

I have followed all the steps creating and activating a form but when I click submit I am sent the .php file I created but as a www. address:

Another matter, I reported some black background, it still appears once in a while but in my site map page I get the same effect in the fonts in the right bellow corner: (I've tried everything to correct this)

This just might get moved to the forms section-Just FYI.

What steps did you use- knowing what steps you followed will help us figure out your problem.

Also- can you copy and paste the php script you used? That would be a big help.

I agree with Andy you have a problem within your .php script we need to see it to be able to resolve your problem. Cut and paste it and e-mail it to the form.
