I'm having problem publishing my page number 4th

and the page I want to publish is

I don't know what I'm doing wrong,I'm doing everything like the first page

You have the wrong link, your page did publish. Here is the link, notice it is a php not an html

what is the different between php and html?
can I change the page to html file?

All pages with forms need to be php, if you change it to html it will not work and you will see html code displayed on you page. If you want to know the difference google them. Good luck.
