One page won't show up

I have made a website and my index page works but i can't get my other page to show up i get an error when i click on the link

This one doesn't http://www.usedatvsalvage.com/parts_request

ALL pages created with Blue Voda that have a Form on them must be published with the ".php" extension to function.

Here's your page, and what the proper link should look like >> http://www.usedatvsalvage.com/parts_request.php

(Always remember to include the proper page extensions in all hyperlinks for all pages, adding the ".html" for all those pages that do not have Forms on them, just as the Tutorial instructs.)
