trying to upload theme in wordpress

I'm trying to upload a theme in wordpress and there is a form requesting this info:

Host Name:

User Name:

Password:

I've put in every combination of my vodahost account that I could think of and I can't get it to upload.

my url is blog.alexbookworm.com. This is where I uploaded wordpress using fantastico.

If you do a simple Forum Search on your own, you'll be able to find many answers without waiting:
>> Wordpress Uploads > hostname?
Hostname = localhost
Username = (the username you use for your cPanel or Blue FTP)
Password = (the password you use for your cPanel Login or Blue FTP)
