phpBB 2 installation

I don't know what to do! I've upload the new template phpBB2 into my website, try to install it and stuck on the first page already!!!

Database Configuration:
Database Server Hostname / DSN: my dsn hosting right?
Your Database Name: My login name in my website?
Database Username: What's this?
Database Password: What's this too?
Prefix for tables in database: phpbb_ (Right?)

Admin Configuration:
Admin Email Address: Can I put any email that I use?
Domain Name: I know this one
Server Port: I know too
Script path: /phpBB2/ (Right?)
Admin Username: Which one? The one for the forum or my website loging?
Admin Password: This too! What the heck?

Please help me, it's too confusing!!!

The above database info is user specified, during database creation.
click the belwo to lean how to create a database...

You might want to think about going to your C-panel click fantastico ( bottom right blue smile ) then scroll down left side to Discussion Boards click phpbb2 the click New Installation the forum & database will be done for you.
