Need help with form

Hi, I made a form for requesting info and where indicated said I wanted email notification. The info is going into the database but I am not getting any emails. I cannot figure out where to go in and edit the form info in case I did something wrong. I'm sure it is right in front of me and I am just not seeing it but a little direction would be appreciated.

I made the form with the form manager in soho. Tell me how to show you what I did and I will. My site is www.jeltraining.com

Ok.. this is for a newsletter and it doesnt send you emails. you have to customize the "ACTION" file it uses for this to send email to you.

Thanks Karen - When I placed it on the page it had asked where I wanted e-mail notifications sent, which is why I thought I was supposed to be getting them. I could never find that page again though. Doesn't really matter much - I can just go in and check it manually.
