I'm using Soho to build my website. I'm having problems in the form area when I attach to shopping cart. My form is complete. Sample on on this page when you add to shopping cart. The form looks ok and has everything I need. I fill everything out. It's when I complete the form and it adds to shopping cart. It's not showing all the fields once it's added to shopping cart. I tried updating, deleting and re-doing the form new and nothing. It's always different. For instance when I add it's not showing the "Up to 3 Family members" information, and the Do you wish to recieve our News Letter Information and a couple other things. When I send it to test the information is not there even though it was completed on the website. Any help would be appreciated.

The form is a really important item I need for my Personalized Childrens Books during checkout and purchase of item.
