Could you can help me with the radio button.
on my mail page I have 5 radios button, when I am publish my page I saw my 5 radios button than when I check, the all button stay checked . But I want have choose to chose one or other one. No 5 the same time.

You have added a refresh code in the page properties html. This needs removing & then re-published.

Not sure what you mean a "printable" form.

If you mean a form that users should download and print, in order to fill it in manually, you need to create either a PDF or a Word form, upload the file on the site, and link to it.

If in example, the file is PDF and is named "form.pdf" , the link to the (uploaded) file should be " http://www.yourdomain.com/form.pdf "

You can make up a form for printing like the one on this page, & then by adding the code below to a html box on the same page for the print button. Best to keep the form around 6-700 wide for easy printing.
