submit form capture?

I like a submit form or slider (I dont mind either one) to pop up after a visitor click the main site a email and name only capture, small one,and a thank you full size page to follow the submission pop up.

Much apprenticed if can find codes for both

Where should I insert that if that already there.

Create pages with the form and the thankyou message.

Add, in your homepage, an "onload" event to popup the form page after , say, 3 - 5 secs.

Hello,

Im bit stuck now. I have created form from the add Form Tool with wizard use.
Two things I want help with..

I can see the created form on my site., but I dont know how to have that form appear for few secs after visitor click the site link,and add thank you page. I tried to Add to event from file, a Thank you page (already saved) to submit button, but cant open from event add Open file.
