I have an existing client (www.snowmantherapy.com) that would like to add a page where clients and potential clients could fill out a profile or survey similar to that found at http://www.5lovelanguages.com/profile that can be given a score or result when completed.
I would need you to write the script and instruct how to include or embed it into a BlueVoda page. Or, I could send you an existing page file to use to create the desired project and e-mail back to me to publish.
What kind of additional information would you need to provide me with a quote for this project?
