Questionnaire tick box options??
Does anyone know how to create a questionnaire, where there could be (for example) three answers to a question. I want the person filling in the questionnaire to simply be able to tick one of the options (such as "yes" "no" "maybe"). They would then complete all questions, fill in their name and click "submit" and the form would then be sent to me as an email with all their questions and chosen answers. Any ideas anyone ????
