Lost username and password for Blue Voda

I am a Blue Voda member, my website is www.modestywebmasters.com. I had to reformat my pc and lost my log in info. I tried to put in a support ticket, but I don't have my username to do that. Is there a way to get my info, I do many websites and I am unable to work till I can get this information.

Put in a support ticket with as much info as you can so they can verify you.

1)Your full name
2)Your domain name
3)The email address that you signed up with

This is enough information.
