need help making printable application form

k, i have my account application form in a word document. I figured out how to make those text boxes so they can fill it out themselves. I want this to be able to be emailed to an address of my choice and for some reason, i cant get my word file to work right on bluevoda! So basically i need to make a page that is printable(printer friendly) or be able to be emailed.

It sounds like you need to create a form the a user fills out and presses the submit button and you receive all the results by email. Is this what you are talking about?

exactly, but my biggest problem is importing my word file, because it has a chart for a fill out section so you can do it on paper!
