How can visitors mail to me from my page???

Hi,
I want to make a page where people can book some of my personal. Is it possible to make a "booking"-form og template (i donīt know the english word exactly) to my site, where people can type in there name, adress, the person they want to book, and so on, and recive it as a mail???
