I would like to create a quiz for viewers.

I need to create a quiz whereas it shows like a species of animal and you choose one of the boxes (choices) it would probly be a list of questions then you press submit and it shows your scores.... Or if you can't do that, they press submit it sends their answers to my email and I publish the high ones and send and email to tell people their scores.
