why doesnt anyone respond to my threads?

i have a page section in my website http://www.viasecurity.ca/account_app.html
i want this fill out form to be able to be emailed to a specific email, and also, i would like to be able to make a "printable" version of this. So i guess some sort of link that is printer friendly for a customer, and another one that is able to be emailed. Could anyone help me out?

there is a bv tutorial which is really good which will show you how to do this. its quite easy to do.

What you ask for is relatively easy but, it takes a lot of work since your form has lots of fields. Also, before going on, please reconsider submitting sensitive data as Bank Account and Credit Card data through a simple, non encrypted connection. If you go on, look at the Forms forum on how to build the form and the php script.
