Thank you. When I clicked on your tutorial address, it came up bigger and I could read the OK. I could not read it earlier. Thank you.

You are now publishing correctly........

but ANY of your pages containing a form must be published with
a .php extension

Open the the 3 pages in BV (one at a time)

Go to page properties and change the page extension to php

Save and Republish the page.

That will get rid of the php code at the top of your pages......................
