Unwanted text on published page

www.fuerteventura-airport-transfer.com/enquiry.html This page when published has garbage (?) text which is not there in the preview. It seems to be attached to the form, as when the form is removed it is not there. I have tried creating the page/form from scratch and it still appears.
Can somebody help me please, I am slowly losing the will to live!

ok in the page properties change the publish as to php. you have published as html. then republish.
the forms work on php and when you publish as html you get that result.

No problem. would suggest a conformation page after submit, and also set one or two fields to be required and you can just click the submit button with out filling any fields out.
