poll set up

I now have an index page up and have added a poll page. http://www.swimmingpoolprofessor.com Can you suggest how to add a poll to this page. It will be looking for names, so I'll need to allow the visitor to write on this page and e-mail it in as well as check box ratings. The compilations from the poll should be viewable in a membership only page which will be developed. Maybe a look at the site's home & poll page will help. Certainly what's there is in it's infantcy, any suggestion would be appreciated
