Values from my web forms disappear.

Hi, I appreciate your contributions to my numerous requests for help.

When a form is filled out on my site http://www.stimulusmod.com, it goes to an email address, but the values disappear. Could someone help me correct this delima. Thanks for being there to help.

Your form Encoding Type is set to text/plain.

Change it to be multipart / form data

Save, republish, refresh before testing.
