I want to add an application form to my site in the hope that people will print it off, fill it in and snail mail it to me. How do Put such a form on my site with my own fields in it ?

Probably the best way would be to make it with word of something and have a download for it, or if you already have a printed version, scan it, save it as an image, and show the image on your site.
