poblem with home page

the home page of my website www.play4pointz.com has a weird code at the top.

does anyone know how to romove this?

That's because you've added a form to the page, which means it is now being published as "index.php" instead of the "index.html" that it was (if you published it before).

The problem is easy to resolve. When someone tries to go to your site, the web is going to look for "index" and will choose the html version of the page. Delete that from the server so that only the php version exists and that should do it.
