Contact Us page not showing online

I'm hoping this is a pretty simple fix.

I have just uploaded my website and all pages except the Contact Us page have loaded and I can access - so I'm pretty impressed - apart from no-one can Contact Us.

I have checked the file name matches the menu bar name at the back end - any other tips to help?

The correct link to this page is http://smileconsulting.co.nz/contact.php

The .html extension was automatically replaced with a .php extension was because you have a form on the page.
