Picture upload

Hiya

im trying to make a picture upload on a form. Ive done lots of forms before and never had a problem. but this one i am having a problem with. it does the usual thning and sends me an email, however it doesnt send me the picture. its on www.petshop.tv then click on the pet of the week thing.

any ideas?
