PHP Form Generator

hi all, please could someone spare the time to give me a hand here, i am tearing my hair out lol.

I have created and installed a form in PHPForm Generator from fantastico but it will not work? i have followed all instructions to the letter.

If someone could point me in the direction of a tutorial or just a few words of advice i would be eternally frateful.

Could you define the problems a little more...are you using FMP ?
is it the forum board ?............................
