Error when trying to INSERT data to the table

Recently the IP address of my site was changed. Everthing worked fine when entering data to the tables. Now, I receive the following error:
Internal Server Error

The server encountered an internal error or misconfiguration and was unable to complete your request.
Please contact the server administrator, webmaster@plasticsurgery.dir2biz.com and inform them of the time the error occurred, and anything you might have done that may have caused the error.
More information about this error may be available in the server error log.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

The file that INSERT into the table has permission 0777.
Please, what should I do?

I see that the problem has to do with the script
header('Location: http://plasticsurgery.dir2biz.com/index.php') ;
BUt what should I do to resolve this?
