Form is not working properly to get to Success Page

Success page ( http://milbyalum.org/successm.html ) is not working properly from my Membership Page -- http://milbyalum.org/membership_info.php. I have this success page set up to access our PayPal payment page. I've done this before and it worked just fine, but not this time.

I'm getting error message and cannot figure out what I have done wrong. Can someone please check this out and help me correct this?

Here is error message:
Warning: Cannot modify header information - headers already sent by (output started at /home/milbyalu/public_html/membership_info.php:1) in/home/milbyalu/public_html/membership_info.php on line 101

The actual form data comes thru to the my email address but the member only sees the "Warning page" indicating that it was not properly submitted. Sure could use some help here. Thanks so much.

Since it seems it's just me helping out in the Forums -- and I'm not a 'Coder' or a User of the Membership script -- you should probably try creating a Support Ticket as "Attn: Hank" ...

Remove the code
<linkrel="shortcut icon"href="favicon.ico">
that you have added in Page HTML, Start of Page. Place it in "Between Head" instead.
