Form Maker Pro Blank Emails

I installed form maker pro and created a feedback form but the feedback emails I receive are blank .
I looked everywhere in the tutorial and couldn't find a solution for the problem.

I'm not sure how this program works but you could try submitting a support ticket as you are not getting many replies here. They might not be able to help you but you never know till you try. HTH
