FORM PROBLEM WITH MULTIPLE FORMS ON SAME PAGE PUBLISHED AS A .php File

I have a page which has 2 froms on it. Both forms published using the BlueVoda form wizard. Each form has a different function but both return email messages to me. Each single form has a unique Subject and Message value. For some reason both forms are generating the same Subject and Message values in the email response to me. This is happening even though each form has unique values. Any ideas on how to correct this problem?
