How do I activate a 2nd contact form on site

I now have a contact page with a form set up with a thank you page. Can I put a 2nd one on the index page? This one would be for ordering a free report to build a list.
Will I need to put a 2nd thank you page and does the code need to be changed? If this is not the way to do please clue me in.

Yes, you can have a second form. The thank you page can be the same (if you are happy with the same thanking message) or different. If you use a second thank you page, make it i.e. "thankyou1" and also set the action of the second form to be "thankyou1.php"
