Hi guys, hope you can help. Have data captcha form on my index page, it captures data to my e-mail, but does not show error page, shows error 404 on net. Cannot find any tutorials on this issue

I cannot believe that there is no one that can help me resolve this issue, has nobody encountered this same problem?
