Strange code appears at top of Index page?

When I updated my add- on website - www.exclusivepurchases.com - it looked great in preview but when published seven lines of code appears at the top of the page. It was published in html and I did try to rebuild the page with the same results!
What can I do?

This is caused because of the form, you need to go to the page properties, then change the extension to php, save and publish the page again. You will then need to remove the index.html page. This will fix the issue.
