Not getting my forms in my email

Hello.
I have tried numerous times to set up a form as descibed in the tutorial section. I am using php to try and email the form to my email account.
The page with the form on it is: www.divehawaiicentral.com/cat.html
The page with the php coding pasted into it is named: action.php

I am still getting an emails delivered to my yahoo email account: sean@reefpirates.com

Can you please have a look and let me know what I need to do to correct this issue and start getting the form in my email? Thanks!

Hi,

your form seems correct. So, if there is an issue, it must be with your action.php . Since it is php, we just can't see the code. If you wish, post it here and we will have a look
