Prevent duplicate entry of data to database?

Hi Folks,

I have several forms that once completed send information to a database or two.

1. I am looking for a method in which to present the information entered on the form for confirmation by the sender.
2. Then following sumission the form is check if information has already beeen entered or not.
3. If it has been entered the duplicate entry is rejected.

Any ideas I would be glad to hear them
